This is a transportation certificate issued by the Baltimore City Health Department for the shipment of a body from Baltimore the Mechanicstown (Thurmont) dated July 5, 1893. This certificate allows Undertaker George J. Smith to transport the remains of a Mechanicstown native,  a 24-year-old gentleman named James Howard Hammett for burial in Mechanicstown. Mr. Hammett evidently drowned on July 4, 1893, and the certificate notes that the cause of death is not contagious. More than likely the remains were transported via the Western Maryland Railway.
